A system of social case recording in which the social worker describes and consolidates all the relevant information obtained about the client system: the progression of the intervention, including procedures used and outcomes, and the interim and concluding prognosis and recommendations. The information is written as succinctly as possible and is based on the social worker's ongoing progress notes, findings before the intervention, and overall conclusion about the case.
