A professional determination about some aspect of a client, such as mental or emotional state; preexisting health condition; or capacity to undertake some new status such as marriage, a job, or a clearance. The information is obtained by the social worker from interviews and tests with the client and provided to a third party. This could be an insurance company, a potential employer, a prospective spouse, an attorney, or others. The information is given only with the client's informed consent.
